  • Serves as liaison between hospice, clients, physicians, referral sources, providers, payers, and community resources to ensure effective coordination of care and supervision of skilled/non-skilled staff
  • Participates in case conferences and acts as a resource to the interdisciplinary team in the identification and resolution of patient/family needs and concerns
  • Provides staff and community education as needed or requested. Participates in professional activities and ongoing professional training to stay current with new developments and technologies and to ensure hospice care is provided according to current accepted standards of practice
  • Participates in hospice quality assurance and quality improvement activities
  • Participates in nursing on call rotation to ensure 24 hour - 7 day a week nursing availability to
  • Hospice patients and their families
  • Must be a registered nurse with current licensure in Massachusetts. Two years of nursing experience is preferred. Home care or hospice care strongly preferred. Current CPR certification is required
  • Ability to perform duties with minimal supervision. Reliable transportation is required
  • Ability to work in a multi-cultural, multi-ethnic environment. Good organization, communication, human relations skills with sensitivity to the unique needs of terminally ill patients and their families
