• Mastery of theories, principles and methodologies underlying population health management and clinical preventative services
• Knowledge of PCMH accreditation standards

• Experience with Nursing workflows and protocol development preferred
• Prior experience in Population Health Management in pharmaceutical or healthcare setting
• Functional understanding of EMR systems
