As Manager of Clinical Process Implementation, I am responsible for oversight and management of the Market Practice advisors who manage and evaluate care coordination processes for an assigned region or market. The position also collaborates with administrators and physician business partners and leadership to provide coordination and collaboration on all activities related to care management functions including changes in process, staffing, programs or regulatory compliance functions. Works on a wide range of activities related to Care Coordination to include planning, analysis and development of action plans between Medical Management teams (Care Coordination, UM, DataRAP, ITC, Clinical Education & Development) and clinics. Serves as a resource on process improvement concepts, principles, strategies and Lean Methodology.

  • Serves as mid-level management between the staff and Sr.Director of Clinical Process Implementation.
  • Participates in the development, planning, and execution of continual process improvement efforts, policies and procedures, and regulatory compliance functions related to care management activities
  • Develops initiatives for continual process improvement of care coordination programs

  • Ensures all new initiatives, policies and procedures, guidelines or other necessary updates are disseminated to the telephonic team and effectiveness is evaluated on an ongoing basis.
  • Solves problems and/or reviews facts, selects the best solution from varying alternatives, and identifies and reports processes or procedures that require modification.
  • Oversees telephonic care coordination staff to ensure compliance with approved standardized guidelines including URAC, NCQA time frames, CMS regulations and contractual agreements.
  • Reviews daily reports utilizing data mining and analytical skills to properly assign members to care coordination teams. Follows policy and procedures for accurate and timely clinical documentations related to care team assignments and other team/member interactions.
  • Collaborates with multiple team members within care model including market management staff to adjust team workloads to maximize productivity and improve patient outcomes.
  • Provides ongoing clinical oversight to telephonic team and reviews care coordination through all aspects of the care continuum.
  • Assists Director of Care Manager Operations with annual evaluations and provides ongoing employee counseling and documentation of performance.Evaluates effectiveness of telephonic
