Evaluate patient progress toward care plan outcomes; collect, assess, and prioritize data; monitor client status and initiate protocols including those for hemodialysis, client care goals, EPA administration, and keeping people alive while activating EMS; analyze hemodialysis indicators for patients with multiple comorbidities; educate clients, family, and support network; help clients feel cared for and respected in their home away from home.

Triage medical and behavioral health needs; collaborate with patient, family and other health care providers to facilitate safe, appropriate patient care including emergency rooms, hospitals, and providers from other facilities; monitor clients and initiate protocols efficiently.

Implement all aspects of ethical care for all patients from admission to discharge; call codes and ensure patient and staff safety in all units of the hospital; develop and follow through with continuity of care in the community after discharge; keep people safe on adult behavioral health units as Charge Nurse, Floor RN and PRN Behavioral Health Technician including court-ordered treatment, general, and chemical dependency units.

Prioritize safety and patient care in all environments in-clinic, on the streets, and in home; cover for nurses and medical assistants on all teams and clinics; analyze, evaluate and coordinate patient progress including integration of medical and behavioral health care.
