Demonstrates competency with interventions and clinical judgment based on unit population and age specific needs. Delivery of a variety of mental health treatment modalities, including psycho-educational groups, milieu management, and behavior modification strategies. Provides assistance to the primary nurse in management of patient needs and support services for effective completion of daily routine.

Complete and conduct psychosocial assessments to determine needs of veterans and family members, be an active participant in the treatment planning process with other disciplines, participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate.

Provide individual, family and group counseling as appropriate, provide consultation/education to veterans and families on community resources, advance directives and VA benefits. Behave as a liaison between VA and community resources, to include marketing and public relation duties as needed, provide case management to veterans and families, as needed, throughout the continuum of care.

Provide a full range of social work services within commonly accepted standards of social work practice which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate. Coordinate treatment and communicate with the care lines to facilitate continuity of care for veterans, maintain data and statistical compilations to comply with JC, CARF, VA and medical center policy and procedures.

Document social work interventions and activities in patient's clinical record utilizing CPRS and ensure appropriate hand off when transferring of patient care, and maintain familiarity and responsibility for all performance measures relating to the HUD-VASH Program.

Participate in formal and informal county agency training programs which provide knowledge of the methods, procedures, and regulations necessary to perform social service and case management. Provide case management to assignments that are limited in scope and difficulty, and work is performed under close supervision. Practice knowledge and skills are acquired, that allow more latitude in judgment and decision making. Educate and advocate for clients and their family systems in stride of family preservation if permitted. Work is performed under the close supervision of a professional social service or administrative supervisor and is reviewed through individual and group conferences.
