Position Summary
The Senior Director of Transitions/Site of Care will lead and contribute to a team of highly engaged colleagues to bring to life our company purpose to help patients on their path to better health. The key focus of this work will be to position CVS Health as the leading provider of services to (1) support people who must transition among sites of care and (2) coordinate enterprise wide efforts to help customers choose highest quality, highly effective, and lowest cost sites of care to meet their needs, including in-home. The goal is to improve the experience and measureable health outcomes and reduce medical spend for both people without chronic conditions, and those with one or more of: Diabetes, Hypertension, Congestive Heart Failure, High cholesterol, Asthma, Chronic Obstructive Pulmonary Disorder, Depression and Anxiety and other common chronic conditions. The chosen leader will be responsible for developing and executing a strategy that delivers approximately $475 million in run-rate medical cost reduction within five years by lowering inefficient use of resources due to suboptimal care transitions, reducing hospital readmissions, improving the healthcare experience, and lowering overall medical costs.
In order to accomplish this, the Senior Director of Transitions and Sites of Care will need to build a high-performing team, establish partnerships with a range of CVS Health colleagues and advance the strategic roadmap through ideation, testing, development and measurement. The leader will make use of innovative solutions from partners and vendors, and the full array of CVS Health assets and capabilities. They will work closely with senior executives and stakeholders from across the enterprise to understand the business and coordinate execution of the strategic roadmap and deliver high-impact solutions.
Required Qualifications
10+ years of work experience
5+ years of experience leading others
3+ years of experience working in healthcare
3+ years of Product Development experience
Preferred Qualifications
Master’s Degree
This person will need to work onsite in Woonsocket, RI
Education
Bachelor's Degree Required
