The Care Manager, Telephonic Nurse 2 , in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Manager, Telephonic Nurse 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.
The Care Manager, Telephonic Nurse 2 employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. May create member care plans. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures.
Licensed Registered Nurse (RN) in the state of Florida with no disciplinary action
Certified Oncology Nurse, OCN
3+ years Oncology care experience
2+ years experience Acute Care experience Med Surg, ICU, Rehab, and/or Hospice
Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook
Must be passionate about contributing to an organization focused on continuously improving consumer experiences
Must be willing to work an 8 hour shift Monday through Friday between 8:30 AM and 7:00 PM Eastern Time
Preferred Location:Dade or Monroe Counties Florida
Alternate Location:Will consider other Locations in Florida
Must be willing to travel to an market office location on a quarterly basis at minimum. May require 5% travel to meet with members at facilities.
Must have a separate room with a locked door that can be used as a home office to ensure you and your patients have absolute and continuous privacy while you work
Must have accessibility to hardwired high speed internet with minimum speeds of 10Mx1M for a home office (Wireless and Satellite are prohibited)
Possession of a valid driver's license and access to a reliable automobile
This role is part of Company’s Driver safety program and therefore requires an individual to have a valid state driver’s license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.
This role is considered patient facing and is a part of Company's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Prefer Combination Adult and Pediatric Experience
Experience with case management, discharge planning and patient education for pediatric and/or adult acute care
Certified Case Manager (CCM)
Managed care experience
