The Company Leonard Davis School of Gerontology, Secure Old Age Lab (Company website) conducts research to enhance the well-being and quality of life for underserved and vulnerable older adults, including educating and providing support to family caregivers. We are seeking a part-time Care Coache to work on the Comprehensive Older Adult and Caregiver Help (COACH) study, a project funded by the National Institute of Justice. This position will be a fixed-term, grant-funded position for a maximum duration of two years. The position is funded by a research contract/grant and may be funded by a subsequent contract/grant. Accordingly, the position is contingent upon receipt of the necessary funding from this or any subsequent contract/grant to continue employment.
The coach will work with family caregivers who are caring for family members or friends who are 65 years or older who have cognitive impairment and /or multiple medical conditions. The project is investigating the impact of implementing a home-based intervention.
The Care Coach will work under the direction of the Secure Old Age Lab Project Manager. Specific responsibilities will include:
Assisting with coordinating and conducting home visit activities with participants to meet research objectives and study protocol compliance: 1) Maintain contact with participants enrolled in the study and schedule appointments for study visits; 2) Prepare materials and present them to participants during home visits; 3) Record post-visit observations in data collection system; and 4) Update participant records in data tracking system, as needed.
Provide project support to the research study including staff, student, and faculty investigators.
Submit timely, accurate and complete activities, reports and other materials, as directed.
Consult with service providers and community agencies on case management issues; research and provide information to caregivers regarding affordable community resources; organize and coordinate community resources, social service agencies, and other psychosocial referral sources.
Maintain a weekly log to document participants served and services
provided to meet project goals.
Provide input on findings and ideas related to interpreting what is observed.
Meet with the supervisor and team on a regular basis.
Perform other related duties as assigned or requested.
Typical work schedule: Monday – Friday; hours can range between 8:00 AM – 5:00 PM, with the flexibility to work in the evenings and/or weekends to allow for home visits when participants are available.
Good listening skills
Comfortable meeting and engaging with clients in their home
Attention to details, observing and recording information
Preferred Education: BA/MS/MSW or some college experience and 1-2 years of work in aging services.
English/ Spanish speaking and writing abilities (not required)
At least one-year experience working with families and/or older adults
Conducting in-person interviews
Background in Motivational Interviewing (MI) techniques
Case Management experience or experience in providing social support services.
Familiar with 65+ adult population.
Comfortable with entering information for post visits.
Driver’s License with a clean record and insurance
Reliable mode of transportation (personal vehicle preferred)
Clean TB Health Screening
Flexible to work evening s and or weekends
